The challenge families face

Choosing the community is the easy part. Everything after it is the hard part.

A single move can touch a lifetime of belongings, the family house, elder law, finances, care needs, and a hospital discharge clock — often all at once. Most families are facing it for the first time.

  • Downsizing a lifetime of belongings
  • Deciding what to do with the family house
  • Understanding elder-law requirements
  • Making financial, tax, and care decisions
  • Managing differing family opinions
  • Meeting hospital discharge timelines
  • Evaluating senior living options
  • Coordinating the move itself

You and your family don't have to do this alone. Our Transition Coordinator will be supporting you through the entire transition.

What we do

A One-Source Solution for Senior Living Transitions

Your Transition Coordinator handles the moving parts and keeps everyone working from the same plan — so nothing is dropped, duplicated, or delayed.

One dedicated coordinator

A single point of contact who serves your family throughout the entire transition — one person who knows your whole situation.

Every aspect coordinated

Housing, legal, financial, moving, and logistical needs — organized, scheduled, and kept moving in the right order.

Barriers removed

We clear the real-world obstacles that delay a move into assisted living, memory care, or other managed care communities.

Certified professionals

Introductions to trusted, senior-focused specialists from our Professional Registry — attorneys, advisors, move managers, geriatric care experts, and more.

The house, handled

Preparing, selling, or transitioning a long-time house when needed — timed to align with the move-in date.

Everyone in sync

We work alongside communities, hospitals, discharge planners, home care providers, and placement agencies — and keep your family informed.

No cost to the senior, the family, the community, or the professionals involved.

We Remove the Roadblocks to a Safe,
Timely Move

We coordinate the non-medical pieces of the transition so families don't have to juggle dozens of calls and details.

Housing Transition

We develop and manage a senior-specific housing plan that aligns timing, family decisions, legal requirements, and market strategy to ensure a successful, profitable, and timely property transition.

Legal & Estate

Provide the legal foundation for a safe transition — establishing decision-making authority, preparing essential documents, navigating elder-law requirements, and protecting the senior's interests throughout the move.

Financial Guidance

Provide senior-focused financial counseling and advice to maximize financial returns, provide the funding for long-term care, and minimize the tax implications.

Moving & Logistics

Oversee senior move managers, packing, transportation, and relocation details.

Care Coordination

Connect with care managers and service providers to support ongoing needs.

Ongoing Support

We stay with you from start to finish — and follow up until you're completely settled.
